Referrals

Operation Bootstrap requires referrals for all our services. If you need a referral, you will need to contact any of the following:

  • School Counselors or Social Workers – contact the school your child attends for a referral.
  • Headstart (715-343-7162)
  • Cap Services (715-343-7125)
  • Church Officials – contact your pastor and ask for a referral.
  • Portage County Health and Human Services – (715-345-5350 – general number) Contact your social worker for a referral.
  • Veterans Services – (715-346-1310)

Emergency Services is located at 1224 Strongs Avenue, Stevens Point, WI 54481.

Services

Food

You can receive up to two weeks worth of food up to once a month. Food can be picked up during regular office hours provided you have a referral. If you are without a vehicle, you can also request delivery.

Transportation

Gas referrals require proof of a valid driver's license and proof of registration of vehicle. You need to provide the make, model, year, color and license plate number of your vehicle to the referring agency when making a request. Gas requests can be authorized up to two times in a twelve-month period.

Bus passes are available on a limited basis for those without a vehicle.

Rx

We are currently working with Metro Market and Home Town Pharmacies. You will need to provide the name of the pharmacy, and name and cost of medication to the referring agency when making a request.

Clothing

Please include items needed along with gender and sizes when requesting clothing.

Furniture

Please be specific on your referral. If you are in need of a used appliance, furniture or mattress, you will be placed on a list. When an item becomes available, you will be contacted by our furniture coordinator.
